Differential Diagnosis :

A-Unilateral :

i-Acute :
-blood clots in legs.
-Acute filaria.
-Rupture of plantaris muscle tendon.

ii-Chronic :
-Varicose viens.
-Celllulitis.
-Chronic filarial.
-Lymphedema.

B-Bilateral :
i-Cardiac.
ii-Renal.
iii-Nutritional.
iv-Angioneurotic.
                                                                
                                         Investigations:

Blood clots in legs is a potentially fetal disease and its clinical picture overlaps with many other conditions, so once clinically suspected ,the diagnosis of blood clots in legs must be confirmed & excluded by accurate investigation.






To detect Blood Clots in Legs :
1-Doppler Ultrasonography "accurate in 80:85 %"
2-Duplex ultrasonography "accurate in 90:100%"
3-Recently Radioactive fibrinogen to detect active uptake by clots
4-Recntly Spiral CT "most accurate"
5-Venography "not done now" ,"it is the gold standard but invasive"


If pulmonary embolism suspected :
1-Sppiral CT or pulmonary arteriography.
2-Ventilation /perfusion scan.
3-Pulmonary angiography.
4-Chest X-ray.
If Recurrent Blood Clots in Legs in young patient :
-measure protien C , S , Antithrombin 3, lupus anticoagulant.
NB : Blood Clots in Legs go to lungs except if there is ASD or VSD plus Eisenmenger $ lead to Systemic embolism.
